路聽Please complete the Sponsorship Request Application Form provided below.
路聽We ask that you complete this form at least 4 weeks prior to your event. Requests received less than 4 weeks prior to the date of the event will not be reviewed.
路聽Only if your application is deemed successful, SHHS will contact you.
