RightRx enables community and hospital medication lists to be easily reconciled

A medical reconciliation software to combine a patient’s prescriptions from local pharmacies and hospitals has been designed at ۲ݮƵ University.

Market Need
Medical reconciliation is the formal process where healthcare providers work together with patients and care providers to ensure accurate and comprehensive medication information is communicated consistently across transitions of care. As local care givers or doctors may prescribe different medications than a hospital, an online repository of information is necessary to avoid duplications or adverse drug interactions.

Technology Summary
The RighRx technology comprises software and connections to data sources that is designed to enable community and hospital medication lists to be reconciled at admission, transfer, and discharge of patients. Since many of the problems arise when local pharmacies and hospitals are not able to communicate prescriptions, this software electronically retrieves and displays records of dispensed medication. By integrating these two systems and their contact information, the best patient medication list is prepared in accordance with Institute for Safe Medication Practices (ISMP) Canada guidelines.
