ANAT course final LAB exam deferral application

听

This form should be used only make a request to defer听the final LAB exam for:

  • Fall semester: ANAT 214,听ANAT 261 and ANAT 315听

  • Winter semester: ANAT 314, ANAT 316 and ANAT 514

*If you do not have supporting documentation for your deferred LAB exam request, you can complete and submit the听PDF icon Student Self-Declaration Form

听

If you need to make a request to defer the听final WRITTEN exam, you must complete the request form in Minerva and follow the policies on the Exams website.
听

  • LAB exam requests will be reviewed听by the Course Coordinator

  • WRITTEN exam requests will be听reviewed听by Service Point (for students in the Faculty of Science) or your Student Affairs Office (for students in the Faculty of Education and School of Physical and Occupational Therapy)
