Academic History Workbook

Applicants (including ÎÛÎÛ²ÝÝ®ÊÓƵ students) applying to the MDCM or MDCM PhD program must submit a workbook. Only International Medical Graduates (IMG 2 & 4) and Med-P Qualifying year applicants are not required to submit a workbook.
  • Download the Workbook provided and complete it with your academic history, including course history.
  • Table 2 of the Workbook should be completed with your entire history of higher-learning (e.g. post-secondary, undergraduate, graduate, etc.).Ìý This includes advanced secondary programs such as APs from College Board, the International Baccalaureate from IBO, CEGEP programs, and the like.Ìý
  • Table 3 need not contain graduate course history.
  • Please note some date formats are not compatible with certain operating systems.Ìý Should you receive an error message, please disregard.

Compatibility: The Workbook was created in Microsoft Excel 2016. The desktop version of Excel is recommended; tables are also compatible with the free version of

Submission

Before submitting your final Workbook, ensure that you rename the file as indicated in Section 6 of the Academic Workbook

You will be able to upload the Workbook in the Applicant Portal after you have submitted your application.Ìý Be sure to include your ÎÛÎÛ²ÝÝ®ÊÓƵ ID on the Workbook.Ìý For new applicants, your ÎÛÎÛ²ÝÝ®ÊÓƵ ID will be visible in the Application Portal, top right corner, within 24 hours of your application. If you have a ÎÛÎÛ²ÝÝ®ÊÓƵ ID as a prior student or applicant, continue to use this ID; it will not change.
